Private coaching gives you depth, pace, and full privacy. Group coaching gives you other people's questions, accountability, and a lower cost. Neither is the advanced version of the other. Fit matters more than prestige.
If you are quietly assuming group is the budget compromise, I want to complicate that. For some people it works better than one-to-one ever would.
Here is the thing I want you to hear first. I see you if isolation is the actual problem, and a private hour would only give you another professional to be impressive for. You are not less serious because you want a room.
I am Cheri Sacks, a registered nurse and a Certified Diabetes Care and Education Specialist. I have watched people thrive in a circle they almost did not join, and I have watched people need a closed door. Both are legitimate.
The short version
Private is for complexity, privacy, or a schedule that will not hold still. Group is for momentum and the relief of hearing someone else say the thing you thought was only you. Cost is a legitimate reason to choose group. It is not a lesser path. Research does not crown a single winner between the two formats.
Both still sit beside your doctor. Neither replaces medical care. Please talk with your physician before you change a medicine, a meal plan, or an activity plan that could drop your sugar. I work with them in both rooms.
What is actually different
Private is your hour, your agenda. We can go into places you would not raise in a room.
Group is a shared hour with a small set of people in similar seasons. You learn from questions you would not have thought to ask. You also have to share airtime.
A good group is small enough that you can hear yourself think. People are in a similar season, not a random mix of every diagnosis on earth. There is an agreement about privacy. Nobody is ranked by their glucose.
A bad group is a webinar with a hundred faces, a leader who talks the whole hour, a ranking of whose numbers improved, or a sales close at the end. If you leave feeling like a lead, it was not coaching. Ask, before you pay: how many people, what is the agreement about privacy, who is in the room, and what happens if I need to step out.
Recognized DSMES is a specific, often covered education program. A coaching group I run is not automatically that. You can have both. You should not be told they are the same product with different lighting.
If you freeze in rooms, private is a kindness, not a luxury. If you perform in private and only tell the truth when someone else goes first, group is a kindness. Neither is a personality test. You can try a discovery call and say, out loud, "I think I need the closed door" or "I think I need other people." That sentence is enough to start.
Partners sometimes want to sit in. In private, we can talk about whether that helps or whether you will edit yourself. In group, a partner is usually a no unless the group was built for pairs. Ask. You get to have a room that is yours.
Show-up is the hidden skill. A cheaper group you attend beats a private package you cancel because the hour felt like another job. If your Tuesdays are already a pile, we pick the lighter container. That is not settling. That is matching the tool to the life. Healing forward, not a more impressive calendar.
You will hear someone else name the 3 a.m. worry you thought was only yours. That moment is the medicine. It is also why group fails when it is too large, too salesy, or run by someone who cannot hold a room.
How I help people choose
Choose private if you have stacked conditions, a story you are not ready to tell in a circle, or a life that cannot make a fixed group time.
Choose group if isolation is part of the problem, if you want the cost lower, or if you do better when other people are walking too.
You can start in one and switch. That is not flaky. That is information.
The letters do not change between rooms. I still coach through PATH: Personalized care, Awareness, Transformation, Health. Private lets us go deeper on the personal part, the story you are not ready to tell in a circle. Group often speeds awareness, because someone else says the sentence first. Transformation is still one doable step, not a ranking of whose glucose improved. Health is still you feeling like yourself, not a perfect number on a shared spreadsheet.
I still work alongside your doctor in both formats. Coaching does not replace DSMES or a CDCES referral when you need specialty diabetes education. The ADA's 2026 Standards on health behaviors still want DSMES at diagnosis and when life changes. A coaching group is not automatically that recognized program. Ask which one you are joining.
It does not replace a dietitian when your kidneys or insulin ratios need medical nutrition therapy. NIDDK's care team page still puts you in the center, with clinicians around you. Coaching is one chair, not the whole table.
Cost, said without shame
Private hours cost more because they are only yours. Group costs less because the hour is shared. That is arithmetic, not a verdict on your worth.
If money is the real constraint, say so in the discovery call. I would rather put you in the container you can keep than sell you a private package you will dread.
Bay Area ranges live in how much health coaching costs. Numbers change. The principle does not: pick the format you will attend.
Recognized DSMES programs can be covered. Private coaching often is not. Ask before you book.
When I steer you away from group
Active eating-disorder treatment, a crisis, or a story that is still too raw for a circle. Those belong in private, or with a therapist, or both. Group is not a cheaper substitute for clinical care.

A moment from practice. A man swore he hated groups. He joined anyway because the private slot did not exist for three months. In week two someone else said the sentence he had been carrying about being a burden. He did not have to perform after that. He stayed. That is not a rule that everyone should join a group. It is a reminder that isolation lies.

The right container is the one you will actually show up to, where you can tell the truth.
