I am Cheri Sacks, a registered nurse and a Certified Diabetes Care and Education Specialist. I coach people in Los Gatos and the South Bay by video across California, and in person on the Peninsula and South Bay by arrangement. Education and coaching, alongside your doctors, never instead of them.

The short version

Video sessions so you are not adding a commute to a body that is already tired. We plan around hills, a packed family calendar, and care for aging parents that nobody put on a slide. A California RN license means I can work with anyone living in the state. The first discovery call is free. No lecture.

Hello. Los Gatos looks restful from the outside. Live oak, a downtown you can walk, trails above the town. The people I meet here are often holding a job, a household, and a parent, and the diagnosis arrived in the middle of that.

Here is the thing I want you to hear first. Looking restful is not the same as having rest. You are allowed to need a person for the in-between.

What this town actually asks of a body

The hills are real. So is the expectation that you still "keep up." The grocery run, the school loop, the quiet decision to stop hiking with friends because the pace changed.

South Bay care is excellent and fragmented. You can have a superb endocrinologist in San Jose and still have nobody for the Tuesday between visits. That is the gap I fill. See building your chronic illness care team in the Bay Area.

The American Diabetes Association's 2026 Standards on health behaviors still want diabetes self-management education at diagnosis, when goals are not met, and when life changes. Life changing is a parent moving in, a job that will not hold still, a body that will not take the old trail at the old pace. That is a clinical conversation, not a character test.

If diabetes is in the picture, living well with type 2 is the map. Walking after meals fits a downtown loop better than a gym you will not join.

PATH in a town that looks like it already has time

I coach through a simple framework I call PATH: Personalized care, Awareness, Transformation, Health.

Personalized, here, starts with the household, not a template. Your hills. The parent who will not "be a burden." The trail you used to do with friends. We do not pretend you have a blank afternoon because the downtown is walkable.

Awareness means we name the load before we add a program. Stopping the hike is information. So is the week you ate in the car between school and the specialist.

Transformation is one small doable step. A downtown loop after the meal you actually eat. A question written down for the San Jose visit. Not five new habits by Monday.

Health is you feeling like yourself enough to stay in the town you chose, not performing wellness for it. Better numbers are welcome. They are not the only scoreboard.

NIDDK's healthy living with diabetes page is plain: talk with your health care professional before you start a new activity or change how much you do, especially if you take insulin or a sulfonylurea. I work with your physician, not instead of them.

A moment from practice. A man in Los Gatos had been skipping the trail and calling himself lazy. We walked the flat downtown block after dinner instead. He kept it. The trail can wait. Tuesday cannot.

Picture a Los Gatos Tuesday that is not a brochure. Morning is a parent who needed a ride. Noon is a meeting that ran. Afternoon is the grocery store because the house is out of the easy food. Evening is the trail you used to do, which you drive past and do not stop, because your legs already did the hills in town. We do not add a 6 a.m. gym to that picture. We pick the downtown block after the meal you actually eat, and we write one question for the San Jose visit so you are not arriving empty. If the parent wants a chair, they get a chair, as long as they live in California. If you want the closed door, you get the closed door. First session is what that hour looks like. Group vs private is how we choose the room.

Sleep belongs on this page too. A packed household will steal it. We treat bedtime as part of the plan, not as a reward after you have kept everyone else comfortable. See type 2 diabetes and sleep if nights are the open loop. Caregiving has its own long piece: caring for an aging parent. PATH holds both without turning you into two full-time jobs.

You can also be the person who looks fine at the farmers market and is not fine at 9 p.m. Both people are allowed in the same body. We start with the 9 p.m. one.

If the trail is the grief, we can say that. Stopping a hike with friends is a loss. PATH does not rush you back onto a grade you cannot keep. Personalized care might be the downtown loop for now, and the trail later, or never. Health is not a summit. If diabetes is new, what a CDCES is is the credential page. If you want video without a South Bay drive, virtual coaching is the statewide door. The free call is still just a call. You do not have to arrive impressive. You have to arrive.

A final thought

You do not have to earn rest by keeping the town's pace. You have to find the pace your body can keep. PATH is how we find it without turning your life into a second job.

You do not need to do more. You need to do what works for your body.
