I am Cheri Sacks, RN, CDCES. Concierge-level coaching for founders, executives, and their households on the Peninsula: the days between the executive physical and the next one. I sit alongside your concierge physician. I do not replace them. I do not diagnose or prescribe.

The short version

Direct access to a nurse coach for people whose calendar does not bend. Built for Atherton, Woodside, Portola Valley, Hillsborough, Los Altos Hills, Palo Alto, Menlo Park, Los Gatos, Saratoga, and nearby. Coverage can include a spouse, a parent, or an adult child. Health in these households is rarely one person's problem. Discretion is structural. No case studies, no named references.

Hello. It is 6:40 a.m. The call with another time zone starts at seven. Three weeks ago you had a thorough physical. It produced a PDF, one number flagged in a polite font, and a follow-up in six months. Since then, nothing. Not because your care is bad. Because nobody's job description covers the part in between.

You are not underserved medically. You are unaccompanied.

Here is the thing I want you to hear first. A six-month PDF is not companionship. You already pay for excellent medicine. This is the hours that PDF cannot sit with you.

What this tier is

Continuity. The plan after the visit. The 10 p.m. text that is not an emergency and is also not nothing. Help translating a specialist's PDF into a week that already has too many rectangles on it.

A BMJ Open review found continuity of care associated with lower mortality across many settings. PubMed 29959146 That is association, not proof that any one service changes an outcome. The signal is still about being known over time. Your physician gives you that at the visit. I am the layer underneath it.

The ADA still treats diabetes education and support as part of care, including when life changes. ADA Standards, 2026, health behaviors A founding round, a parent in decline, a body that will not perform on command: those are life changes. Education belongs there. A step contest does not.

PATH under the executive physical

I coach through a simple framework I call PATH: Personalized care, Awareness, Transformation, Health.

Personalized, here, means the household, not a generic "executive wellness" deck. Your 6:40 a.m. call. Your spouse. Your parent. Your PDF. We start there.

Awareness means we look at the months between physicals, which is where most of the story lives. A flagged number with no Tuesday plan is a file, not care.

Transformation is one doable step that can live inside a calendar that does not bend. Not a retreat. Not a ranking. One lever.

Health is you feeling like yourself enough to stay in your own life, not a trophy lab. Better numbers are welcome. They are not a personality, and they are not content.

A moment from practice, anonymized on purpose. A founder had a perfect physical summary and a 10 p.m. glucose he had not told anyone. We did not add a slide. We picked one evening walk after the late meal, and we told the concierge physician, with permission. He kept the walk. The PDF stayed in the folder where it belonged.

I offer diabetes education and coaching, one-to-one, and group when that is a better fit. For this tier, private is usually the room. The first conversation is a free call. You can book that here.

What "unaccompanied" looks like at this altitude. You can get a same-week specialist. You cannot get a person who knows that the flagged number happens after the 10 p.m. call. You can fly for a second opinion. You cannot find 20 minutes that are not already promised. PATH sits in those 20 minutes. We translate the PDF into one evening lever. We include the spouse or the parent if they live in California and they want in, because health in these houses is rarely one person's problem. We do not write a case study. We do not need your name.

If the Peninsula is the geography, Palo Alto and Los Altos are the town doors. If the first hour is the mystery, what to expect. If the question is who owns the medicine, coach vs dietitian vs doctor. Continuity is the product. Discretion is the container. Your concierge physician stays the physician.

A 10 p.m. text that is not an emergency is still allowed to be a text. We will decide together what belongs in a message and what belongs in the next session. I am not an on-call hospitalist. I am the layer under the physical, for the hours the physical cannot sit with you.

Atherton, Woodside, Portola Valley, Hillsborough, Los Altos Hills, Palo Alto, Menlo Park, Los Gatos, Saratoga: I will not flatten those into one brand of wealth. I will say the gap after the physical is the same job. Households here often need more than one chair. A spouse. A parent. An adult child who is the backup. PATH can hold more than one person without turning anyone into content. No named references. No case studies. If group ever fits, we talk about privacy first. Private is usually the room.

Cost is a fair question. Ask it. This tier is continuity and discretion, not a poster campaign. Your concierge physician stays the physician. I stay the Tuesday. You do not have to perform wellness on the free call. You can say the 10 p.m. number out loud and we will start there. There is nothing to apologize for. You have to arrive. That is enough for a start.

A final thought

A six-month PDF is not companionship. PATH is how we sit with the hours that PDF cannot.

You do not need to do more. You need to do what works for your body.
